On the day of your visit
- Identification mandated by law:
Effective November 1, 2009 we are complying with the red flag requirements for your protection. Please bring in a photo ID and current insurance card to each appointment.
*If your photo ID does not have your current address, please bring a utility bill in your name to validate your new address.
- H1N1 SFPA Practice Policies:
Because of the spread of the H1N1 flu we are taking every step possible to protect the health of our patients and staff. Please use this link to review our policies before you arrive for your appointment.
- Late Policy Statement:
If you are running late, please call us at 415-202-1200, as we may have to reschedule your appointment.
- Reminder for Ultrasound Patients:
We would like to remind you that your ultrasound is an important medical examination that requires a quiet environment for the sonographers to accurately assess the fetus. To assure that we can maintain this environment we ask you to use these guidelines:
- You are welcome to have 2 other adults accompany you to your appointment.
- We find that children under 4 don’t enjoy the exam and become distracted. You may bring 1 child, but please be sure that an adult is available to take them out of the exam room if needed.
- If you do invite other people to your exam it’s important that everyone is here at your appointment time. We can’t wait for family members who are late to start the exam.
- There are no cell phones allowed in the exam room.
- No photography or video is allowed unless the physician is in the room.
