Preparing for Your Visit

In order to make your visit to SFPA as efficient and as personalized as possible, we ask that you complete the registration paperwork relevant to your specific appointment prior to your appointment. If you have completed your paperwork prior to your appointment, please arrive at least 15 minutes before your appointment time. If you have not completed your paperwork prior to your appointment, please arrive 30 minutes before your scheduled appointment time.

Please also be sure to bring the following items with you.

• Current insurance card
• Referral form from your doctor’s office

Ultrasound examinations and procedures require a full bladder and you will be instructed about how much water to drink prior to your appointment by our scheduling team and our genetic counselors.
